I found this in a box of vinyl records from an old radio station that closed down 50+ years ago. The order slip was still inside the box. Because this was a radio station order, the records came in brown paper sleeves, no cover sleeves.
The record is in excellent condition and does play. I don't know WHAT kind of record it is, but we played it on an old Admiral 1954 5D3 record player. If you know how I can determine what kind it is, by all means, educate me. I have over 100 more records like this lol. And it would be awesome to get rid of them all because they are heavy as hell.
