You have a very nice item. The back is St. Christopher (I believe) who is normally depicted with the Christ child on his shoulder. This is a Catholic medal. It is to remind us of those saints - what holy things they did in their life - and how we might strive to do the same.

My guess is because it not only has St Christopher (patron St. of Travelers) and the Pope on it. It was a medal purchased by someone that made a pilgrimage to see the Pope in Rome or something like that. Not being Catholic and not have a really good view of the legend on the Pope side of the coin I can't say more than this.
