There is still the original plastic over the outside faceplate and there are a couple of TINY scratches but I had to look really close to find them. Does not come with charger, I am sorry about that I can't find it, I also do not have a manual but you can download it here http://support.t-mobile.com/docs/DOC-1203

jenny: yes you just put the sim card in the phone. treatede: plans are based on your area so you would have to go to the website and put in your zip code.
