It's just one and the spiked top is the only 14G one I have. The hoops are not huge like septum ones but you can use them for it, it's more discrete. The balls are closer together so they look cute on lips, even on angel bites I have a black pair in mine. The labrets are not for those people that just got their piercing these are the standard length they aren't longer. I had my cartilage for awhile and I had a labret in it because the earring hurt so bad to sleep with so it works for them too [: super cute!
